Degenerative-dystrophic diseases of the joints and pathologies of an inflammatory nature often accompany pain, an increase in body temperature. And an effective drug is required that can eliminate unpleasant symptoms. With such pathologies, Movalis rectal suppositories come to the rescue. They relieve pain, lower fever and fight other signs of inflammation.

The composition of the metabolic agent

The active component of rectal suppositories is meloxicam. A substance from the group of non-steroidal anti-inflammatory drugs (NSAIDs). Meloxicam comes from the oxycam group and has a strong effect against inflammation. It was first patented in 1995 by the German company Beringer Ingelheim. It soon became available in more than 40 countries.

Auxiliary components of suppositories are suppository mass (suppository BP) and glyceryl hydroxystearate of polyethylene glycol.

Suppositories in 7.5 and 15 mg are available. They have a yellowish-greenish tint, smooth. From below, each suppository has a recess similar to a funnel.

Pharmacological properties and pharmacokinetics

The drug has several therapeutic properties:

  • analgesic;
  • antipyretic;
  • anti-inflammatory.

These properties are due to the specific mechanism of the active component. Meloxicam is an inhibitor of prostaglandins, and they are mediators of the inflammatory process. And thanks to the selective inhibition of COX-2, a similar process directed to prostaglandins occurs directly in inflammatory foci.

At the same time, the process of suppressing prostaglandins on the membranes of the gastric mucosa is markedly reduced. Inhibition of COX-2 not only increases the therapeutic effect of the drug, but also minimizes the risk of gastritis and other gastrointestinal complications.

Important! Meloxicam at the doses recommended by the doctor does not increase the duration of bleeding, unlike other NSAIDs (Naproxen, Diclofenac), which increase platelet aggregation time, resulting in an increase in bleeding duration.

NSAIDs are well absorbed from the intestines. Its maximum effectiveness occurs after 5 hours (full absorption time). During the distribution process, the drug binds well to plasma proteins. It binds to albumin 99%. It also infiltrates into the inter-articular fluid (1/2 plasma concentration).

The main processes of metabolism of the active substance occur in the liver. Metabolites of meloxicam are mainly excreted in feces and urine. About 5% of meloxicam is excreted unchanged.

Sometimes pharmacokinetics may vary. This applies to cases of renal failure (severe). For this group, the dose per day is reduced by 1 \ 2. It was also noted that in young children (with normal body weight), Cmax in plasma decreased, and the clearance of the active component increased.

Why are Movalis candles prescribed?

In some countries, the purpose of the drug varies, depending on the disease.

In the Russian Federation, the evidence is as follows:

  • rheumatoid arthritis;
  • osteoarthritis and osteoarthritis;
  • Ankylosing spondylitis (ankylosing spondylitis).

In medical institutions of other countries, the drug can be prescribed for the treatment of any inflammation in the spine and joints, accompanied by pain and fever. These are brachial periarthritis, sciatica and other diseases of the musculoskeletal system.

In urology and gynecology, rectal suppositories are used for inflammatory diseases accompanied by acute pain (for example, with adnexitis). Movalis is also prescribed for the treatment of primary dysmenorrhea (in this condition, there are no pathological changes in the pelvic area).

Instructions for use of the drug

The drug in candles is equivalent to the medicine in tablets at its dosage. Therefore, if the patient was prescribed Movalis in tablets of 15 mg per day, then the dosage in the suppositories will be identical.

Suppository therapy can be used as an independent type of treatment, or as a combination therapy (after stopping a powerful pain syndrome with meloxicam injections or in combination with other drugs).

The standard dosage of the drug is 7.5 mg. If necessary, the doctor can adjust it (depending on the course of the disease). The maximum allowable daily dose of meloxicam is 15 mg.

Important! The course of treatment for NSAIDs is individual, it can vary with different severity of the inflammation process.

DiseaseTherapy features
Ankylosing spondylitis (ankylosing spondylitis)Treatment begins with 15 mg per day. Gradually, based on the apparent improvement in the patient's condition, the dose is reduced to 7.5 mg per day.
OsteoarthritisThe therapeutic dose is 7.5 mg. If necessary, increase it to 15 mg per day.
Rheumatoid arthritisTreatment starts with 15 mg in knocking. Gradually, based on the apparent improvement in the patient's condition, the dose is reduced to 7.5 mg per day.

Specialists recommend using the medicine before bedtime. To do this, you need to empty the colon or make a cleansing enema. After - enter the suppository into the rectum and take a lying position. Since the active component is absorbed within 5 hours, it is this time that is worth spending lying down.

If you experience unpleasant phenomena in the rectal area (itching, burning, pain) that do not go away for a long time, you should consult your doctor.An allergic reaction to suppository components may occur. In this case, you need to choose an effective analogue.

During pregnancy and breastfeeding

Movalis is not recommended for use in pregnant women and with HB.

Reasons why you can not use meloxicam pregnant and lactating:

  • when taking medication in the first trimester, there is a likelihood of loss of internal organs through the non-fused abdominal wall (gastrochisis) and the development of heart defects;
  • inhibition of prostaglandin synthesis can provoke a violation of the development of the fetus in the mother’s womb and even cause his death;
  • when taking medication in the third trimester, there is a possibility of impaired renal function and the development of renal failure in a baby;
  • meloxicam provokes spontaneous bleeding in pregnant women, weakening of the strength of labor.

The active component is able to suppress fertility in women. Therefore, patients planning to conceive a child should refrain from treatment with Movalis.

Drug interaction

Movalis rectal suppositories are not used with a number of drugs and substances:

  • antiplatelet agents, thrombolytics, oral anticoagulants, SSRIs (increased risk of bleeding) and heparin;
  • glucocorticoids and other COX inhibitors, including salicylates (there is a risk of bleeding and ulceration of the gastric mucosa);
  • methotrexate (NSAIDs are able to increase its concentration in plasma);
  • lithium preparations (NSAIDs increase the concentration of lithium in the blood plasma, which leads to an increase in its toxicity);
  • ACE inhibitors and other antihypertensive agents (meloxicam reduces their effectiveness);
  • diuretics (there is a possibility of impaired renal function, constant monitoring of the urinary system is necessary);
  • cholestyramine (provokes the rapid elimination of meloxicam from the body).

Contraindications, side effects and overdose

A metabolic agent is contraindicated in the following conditions:

  • intolerance to the active substance or other components of the drug, as well as intolerance to NSAIDs
  • aspirin asthma (history);
  • pregnancy and breastfeeding;
  • a history of gastric and intestinal bleeding (or perforation) associated with the use of NSAIDs;
  • children under 12 years old;
  • active / recurrent peptic ulcer (or its perforation);
  • severe hepatic insufficiency;
  • severe renal failure, chronic renal failure in patients without hemodialysis (with creatinine clearance below 30 ml / min), with progressive renal pathologies;
  • with gastric, intestinal, cerebrovascular bleeding and other disorders of blood coagulation.

Contraindications to the appointment of rectal suppositories are also proctitis and a history of rectal bleeding.

With caution, it is worth using the medicine for patients with alcohol dependence, with gastrointestinal ulcers, with coronary heart disease, peripheral vascular diseases, as well as with the simultaneous administration of methotrexate, antiplatelet agents and anticoagulants and long-term treatment with the drug from the NSAID group.

Side effects when using Movalis:

  1. Common: headache, abdominal pain, nausea, vomiting, mood changes, vertigo.
  2. Occurring infrequently: immediate hypersensitivity reactions, angioedema, drowsiness, dizziness, anemia, confusion, tinnitus, dyspeptic symptoms, stomatitis, gastritis, changes in the clinical parameters of renal and hepatic functions, delayed ovulation in women.
  3. Rarely encountered: conjunctivitis, colitis, esophagitis, bullous dermatitis, photosensitivity, in the analysis of blood - a change in the leukocyte formula.

Symptoms in case of an overdose: nausea, vomiting, impaired consciousness, drowsiness, abdominal pain, GI bleeding, changes in blood pressure.

Therapy: rinse the stomach, conduct symptomatic therapy to maintain vital functions.Since cholesterol increases the rate of elimination of meloxicam, it can be used in the treatment of overdoses.
